Correcting a mistake in your passport
Passports are no longer amended. In order to change information on your passport, you must submit a new passport application. If you meet the specific criteria outlined below, you can use the no-fee passport amendment form, Form DS-5504 (PDF 976 Kb), to apply for a new passport. Otherwise, you would need to use the form DS-11. In both cases you'll need to make an appointment to submit your application.
All U.S. Citizen Services with the exception of Federal Benefits services and applications for adults over 18 applying for their first passport are provided at the Consular Agencies in Punta Cana and Puerto Plata. For your convenience, we recommend that you use the Consular Agencies for services if they are closer to your geographical location. For example, Puerto Plata provides services to Santiago, Sosua, Cabarete, Las Terrenas, Samana and any of the cities along the North Coast. Punta Cana provides services to Bavaro, Higuey, Bayahibe, La Romana and any of the cities along the East Coast. The Agencies tend to have shorter wait times and be less crowded.

If applying for amendments complete the following form: Form DS-5504 (PDF 976 Kb).
This form should only be used for making a data change for a previously issued, currently valid U.S. passport WITHIN one year from the date of issue:
- When there is a change in your identifying information, such as a name-change due to marriage, divorce or by court order; or
- To correct a Passport Services' printing error.
In order to have data in your passport changed or amended, you will need to present evidence in support of the proposed change.
There is no fee for this service. Processing time is approximately four weeks.
