Consular Outreach Program in Santiago

The U.S. Citizens Services unit offers notarials, passport renewals, and federal benefits services in Santiago at the Dominican-American Cultural Center Library located at Avenida Estrella Sadhala #101.  These services are available the first Wednesday of alternate months, by appointment only. 

Appointments made for the Santiago Consular Outreach dates listed below are held in Santiago only.  The Santo Domingo Consular section does not receive applicants on these days.

In Santiago we accept pesos and dollars, cash only.  See our Schedule of Fees.

To schedule a federal benefits appointment please telephone 809-731- 4371 between the hours of 1:30 pm and 4:00 pm, Monday through Thursday, or 7:30 am - 11:00 am Friday.

Notarial or passport renewal appointments must be made online.  The steps are simple:

Step one: Choose the day and time of your appointment via our online booking system and receive an immediate confirmation.  If you want a notarial appointment you are done!  If you are renewing a passport, continue to step two below. (Please note that all appointment confirmations will show notarial service.  This is standard even for passport appointments).

Helpful Hints for Appointments:

Make your appointment early. Slots can fill up quickly.

If you don't have an email address for the required field, just enter none@hotmail.com.  We like e-mail addresses though because they help us notify you should there be unforeseen circumstances that affect your appointment. We do not share them and they drop out of the system after the appointment date.

Print out a copy of your appointment confirmation and bring it with you.

You can also cancel an appointment through the online booking system.

Step two: Complete the proper passport application.  New electronic passport application forms for all passport services, including passport renewals and adding visa pages, are now available. Please complete your passport application on-line using the Passport Application Wizard.  When you have completed the form - providing all of the requested information - print it out and bring it with you to your appointment together with either the required supporting documents for minors (PDF 220 Kb) or the required supporting documents for adults (PDF 189 Kb).  Based on your responses, which must be accurate, your computer will print the completed application on the correct form. Applications must be printed on separate sheets of paper; do NOT print this form on front and back of the same sheet.

Helpful Hints for Passport Wizard:

Be sure to select the "Create Form" button at the end of the application, print out the completed passport form, and bring it with you to the appointment.

You will have to list a zip code in the electronic form, even for overseas addresses. For overseas addresses, use "00000" for the zip code.

Ignore pages one and two of the printed instruction pages, as they do not apply when applying for your passport in the Dominican Republic. Follow the instructions on this website.

If you still have trouble using the Passport Application Wizard you can pick up passport forms at the Embassy's U.S. Citizen Services Unit, the Puerto Plata Consular Agency, or download blank forms from the internet at http://travel.state.gov/passport/index.html
