What forms do I need to fill out to apply for a Non-immigrant visa?
Every applicant for a non-immigrant visa needs to complete and bring the electronic non-immigrant visa application (DS-156). This form needs to be completed on the internet and printed to bring to the Consulate on the day of the interview, it cannot be sent by internet.
Every applicant should fill out the form (DS-156) as honestly and completely as possible. You should enter the information requested into the appropriate spaces and make sure that all the information is correct. Please be especially careful to select the correct information in drop down boxes such as those for nationality, the code for Dominican Republic should read DOMR on your form. For minor children that do not have a national id number, do not write "minor" in the national id question, this question should be left blank. When you finish completing the form, press the continue button at the bottom of the screen, so that the form will appear in Adobe.
Once the form appears on your screen, review it for accuracy, change incorrect information and print it. There are three pages, two with your information and one that has a bar code. The bar code on the third page contains all the information you have written in your application and will help us accurately document your information. If the barcode is not a perfect rectangle, print it again. Sign the form on the bottom of page 2.
If you are a male between the ages of 16-45 you must also complete the DS-157.
